PointCare Hospital Map

Taylor | PointCare | October 2026

As of 20 Oct 2026

Taylor, here is the hospital map we promised. It ranks the safety-net hospitals in Medicaid expansion states by how many inpatient days Medicaid pays for, and it names ten revenue cycle and finance leaders to start with.

Every count comes from public files, and the sources sit under each table.

Why January matters to a hospital

For Medicaid applications made on or after January 1, 2027, coverage for expansion adults reaches back only to the month before the application, down from three months, and for everyone else it reaches back two months.

For an expansion adult, that takes away two of the three months a hospital has today to turn a self-pay stay into a paid Medicaid claim. Expansion adults also move from yearly renewals to renewals every six months, starting with renewals scheduled on or after January 1, 2027.

The cut

Medicaid share is the share of a hospital's inpatient days paid by Medicaid, managed care included, as the hospital reports it on its Medicare cost report (Worksheet E, Part A, line 31).

The bed floor keeps small rural sites off the list.

Psychiatric, children's and women's hospitals are left out, by name, by CMS hospital type or number, by the hospital's own description, or where psychiatric unit days are most of its inpatient days.

Each count is out of the row above it; the last row groups the hospitals that clear the cut by health system. Each hospital is counted once, on its latest report in the CMS cost report file for fiscal year 2024. Exclusions remove PointCare's own named customers and partners and a short list of our own; none matched a hospital here. A campus another hospital on the list has absorbed is counted once, in that hospital's row.

The ranked list

Ranked by Medicaid inpatient days, highest first. Medicaid inpatient days are the Medicaid share times the hospital's total inpatient days on the same cost report. System is the health system the hospital belongs to, where one is listed.

Who signs at a hospital

Revenue cycle and patient access usually agree first, then the CFO signs.

RoleWhere they sit in the decision
VP of Revenue CycleOwns the money from registration to final payment, self-pay conversion included. Usually the champion.
Director of Patient AccessRuns registration and insurance checks at the front door, where coverage is found or missed.
Director of Financial Counseling or EligibilityLeads the team that screens self-pay patients and files Medicaid applications. The daily user.
CFOSigns larger contracts and answers for uncompensated care and bad debt.

Ten people to start with

Revenue cycle, patient access and finance leaders at ten of the hospitals and systems above, one an organization. Where gives the city of that hospital. Work emails and phone numbers checked on Oct 20, 2026.

Where to start

Lead with January

Coverage found at registration keeps the claim. Coverage found weeks after discharge, on an application made on or after January 1, 2027, may come too late to bill. That is the opening for a revenue cycle leader.

Start at the top of the list

The hospitals at the top of the list are where Medicaid pays for the most inpatient days, so each self-pay patient who qualifies is worth finding before discharge.

Bring the health center proof

A hospital CFO will ask where this has worked. PointCare's published health center results answer that. Tell them in hospital terms, claims paid and bad debt avoided.

Built from CMS hospital cost reports, the KFF Medicaid expansion tracker and the AHRQ Compendium of U.S. Health Systems, all public.
